Here are some of our most common private fees
(these are subject to change, and some accrue HST)
Missed appt or more than 10mins late for appt
also includes cancelling an appt with less than 24hr's notice
Therapist note for insurance
eg. physio/RMT/orthotics
Generic sick note
this is a basic signed note, not a specific off-work assessment form
Ear syringing
eg. for wax
Transferring reports/records
Transferring your report/result to you or a non-physician
the 1st 20 pages of a single report/result
Transferring your notes to a physician
the 1st 20 pages
Transferring notes beyond the 1st 20 pages
per page fee
Insurance Report (APS - Attending Physician Statement)
usually this is paid by your insurer instead of you
Medicolegal Report
often this is paid by your lawyer instead of you
Other private fees
always check with your doctor before booking an appt for these
Private consultation
this includes if you come for your appt with an invalid OHIP card, or if you need a non-OHIP consult such as a travel health assessment
Private prescription, includes for insurance purposes
travel vaccine, eg. Yellow Fever, but does NOT include a travel health assessment
Other forms, by the time taken to complete
eg. short-term disability forms, immigration forms
Pre-employment physical/assessment/forms
always check with your doctor first if they are able to complete it
Full Driver's Physical
always check with your doctor first if they are able to complete it
Only Driver's Medical Form
always check with your doctor first if they are able to complete it
Faxing/sending prescription
picking up your prescription from our site is always free
